In Highland the service is currently being used to support people with Asthma, High blood pressure, Chronic pain, Chronic obstructive pulmonary disease (COPD), Covid-19, and Heart failure with more services being developed and planned.
Connect Me services are free, convenient and easy to use, with a choice of communication methods including text messages, emails and automated phone calls.
Patients are signed up to Connect Me services by their healthcare professional (GP practice or specialist nurse),
